Posted By Stuff4blokes Frank, you could use the " did you ever hear anything so daft as this?" approach by recounting and anonomyzing an accident in a lighthearted way, highlighting the human error, followed by a couple of audience stories. This naturally leads on to how they could have been prevented by a different management system.

When you start talking, tell 'em you were taught always to start with an "icebreaker".... Breaks the ice quite nicely, and (depending on the audience) can often be used to give a taster of what you're going to talk about... Steve
