Frank, you could use the " did you ever hear anything so daft as this?" approach by recounting and anonomyzing an accident in a lighthearted way, highlighting the human error, followed by a couple of audience stories. This naturally leads on to how they could have been prevented by a different management system.

Get everyone to exchange clothes with the person to their left. Been to many parties like this and it's great fun.

A good one I have seen but not yet tried is to get them to create a list of song titles that apply to health and safety, competitive and enjoyable and might even just leave a subliminal message....

If you're presenting to a UK audience, a good one I saw at a FoodNorthwest gig was to get a colleague to do the fire precautions bit in a different language.
Set the scene for a safety discussion very well, I thought!

Put it up on the projector as your delegates are arriving. When you start talking, tell 'em you were taught always to start with an "icebreaker"....
Breaks the ice quite nicely, and (depending on the audience) can often be used to give a taster of what you're going to talk about...
