I have just had a report of a jack handle hitting someone on the head.(big lump).
It appears that the ratchet had not been fully engaged and it rotated hitting him on the head.
We are examining the jacks to see if there is any mechanical failure. If it isn't and its human error (poss training issue) does it still come under:

Reportable dangerous occurrences
(a) Collapse, overturning or failure of load-bearing parts of lifts and lifting
machinery

YEs, reportable.

A lot of accidents ARE caused due to human error.

P.S. that was if the injury caused over three day off work etc.

Not for overturning machinery etc. (as described in your post) - In that case, it won't be.

Definately not as a dangerous occurance. The part of Schedule 2 of RIDDOR you refer to is lifts and hoists as in passenger/goods lifts or hoists

It's debatable. Human factor may always be there but 'error'? Not always.

For instance, I was once bitten by scorpion in my garden one summer whilst I was sitting on a chair in our garden preparing for my exams. It was only then we relised that there were scorpean present there.
