Hi
I am attempting to assess the status of the health and safety culture when my workplace. Could anyone provide an example of their safety culture questionnaire, in order to get me started on the right track.

I should be quite content to share examples of models of safety culture with which I'm familiar if you get in touch. Some of them are proprietoral, e.g. the brilliant Performance Culture cards published by Saville Consulting, and some I've designed myself.
'Organsiastional culture', like 'occupational personality', is an unavoidably loaed concept.
At the same time, I would draw your attention to two facts. Firstly, depending on the character (and climate) of your organisation, as well as your explicit purposes, there may well be much more effective ways of assessing the culture of your organisation. Secondly, a questionnaire without reliability and validity data is likely to become harmful as Chris Argyris, now of Yale Univeristy, first illustrated in 1970.

I has used the questionnaire available from the the Energy Institute, originally developed by Shell, the programme is called Hearts and Minds, information of which can be found on energyinst.org.uk/heartsandminds.
we use it as part of our management and supervisory training courses.
