Two separate but similar incidents have landed on my desk this week:
1) Wages have brought through a self-cert in which they say the injury was caused by an accident at work,. No accident reported in accident book. Turns out that he has aggravated an old injury by "walking on rough ground carrying a load".
2) Wages bring a sick note through for someone who has just returned from holiday worked one day and then gone off with sprained hamstring and back. Again no accident report.
I am in a dilemma as to report these under RIDDOR albeit with an explanatory note.

Can you please explain why you think an accident is reportable because an employee "claims" to have sustained that accident at work?

How many incidents do we (safety professionals) witness at work.. hardly any… but we always listen to an employee who approaches us and claims that he had an accident etc etc. we log that incident accordingly… Similarly if an employee claims to have sustained a reportable injury at work, it becomes reportable. Employee’s failure to report ‘as it happened’ can be added into RIDDOR report.

I hear what you are saying and I understand your caution. However, legally, the incident only becomes reportable if and when the employer is satisfied that it did happen out of or in connection with work.
I spend a lot of time each year advising clients on what is and what is not reportable under RIDDOR as it can have significant impact on e.g. insurance premiums, tenders, etc.
RIDDOR is as badly drafted as the Fire Precuations Act 1971 and it should go the same way as that Act.
