I am looking to update the companies pre-work health check for employees.

The health check is part of on site inductions, so the HSE dept. understand each particular persons health history.

Any help would be apprecieted

Can I ask - is this not all managed by your Occi Health provider? Why would (and how can) H&S Dept understand folks medical history?

When the questionnaires are sent to the Occi H P, they then provide recommendations re 'fit' or 'fit with restrictions' or 'not fit'.

I can help with what we use, but it is really only relevant to us - after our risk assessments.

I would look at the other way from the previous thread and would grasp the opportunity to look at what occ health ask for. There are obvious issues with Data Protection - so if you are looking to redesign the form, rather than gather the info I would look at your risks and identify what occ health need to be looking for - so if you use FLT drivers - they will need questions orientated to that, lone workers would neeed questions orientated to that, etc.

Occ health are great at doing what they do, once they have the info, but quite often, especially when it is outsourced, they ask generic questions which are not geared towards your risks.

Tell OH what the risks are and ask them what they need. Hope this helps.
