Posted By Canned heat I am currently reviewing the way we pre-qualify our subcontractors as I believe the current criteria is not sufficient. On our new screening forms I have included things such as: Can they include suitable risk assessments/method statement, references, training records etc etc would anybody have an example form or format I could possibly look at please to help me look for anything i may have missed or for improvements. any help appreciated

Posted By P. Reynolds A good place to start would be appendix 4 of CDM 2007, we used the criteria set out in this document as part of our competency assessment prior to the contractor being used.

Posted By Barrie (Badger) Etter Contact me with subject line marked IOSH. I'll send you a copy of our supplier questionnaire. We went through the same process a while back and ended up amalgamating half a dozen forms almost doing the same job. As indicated earlier CHAS is a good source of info and our form is based around their info. Badger
