I'm running one for safety observations, like near miss reporting. I ask them to complete a form which is geared towards see what actions they've taken to make improvements/corrective actions etc. A committee then sit down and review the entries against predetermined criteria. The ones which fit best are given high street vouchers.

Maybe a slight deviation of the topic, however, when I lived in South Africa, I was employed as Health and Safety Officer with the Western Cape branch of a national construction company. We used to have an inhouse annual safety competition. The prize was the floating trophy with the name of the winning site engraved on it.

A few years ago I ran a Hazardous Observation reporting scheme where every quarter a team choose the best three observations submitted. The award was a very small monitory token i.e. $50 book / record token or similar but what was different about this scheme was that the small award was accompanied by a letter from the Vice President summarising the observation submission and how the corrective action has improved safety and thanking the individual for their efforts. Many recipients commented on how 'chuffed' they were to get a letter from the top man. Leadership commitment - it makes a big difference.

The company I used to work for had suggestion boxes around the building for people to drop their ideas into (not just H&S). We had a committee who would decide on the best idea on a monthly or quarterly basis, and the best one judged on an annual basis.
Monthly awards ranged from small monetary prizes, reserved car parking by the director places, a small trophy, vouchers for cinema, use of company box at football stadium etc. The annual award was often based on monetary savings to the company as well as safety improvements - if a tangible saving could be directly attributed to the implemetation of an employee's idea they were entitled to a commision on that saving (0.5% on a £100k saving entitled the employee to a £500 reward).
The key to any innovation/reward strategy is communication - it is imperitive that employees know their ideas are being looked at and considered - even if not implemented and we would hold regular catch up sessions to run through the thought processes behind the commitees decisions. The more involved the employee feels, the more they contribute. Look to include posters, email reminders, pay slip inserts etc to help promote any campaign you initiate.

Nephin, I had great success at a company I once worked for with a safety initiative scheme. I became aware that the safety champions around the business were implementing some great ideas without really getting any recognition. I developed an arrangement whereby they applied to me for recognition and I scored their initiative against set criteria. The points were then shown on our Health & Safety Monitor which was reviewed by Senior Managers during performance review meetings on a monthly basis. Bearing in mind that the company operated in 6 different countries so that verification of initiatives was only possible occassionally during audit visits, the basic process was this: - 1. Safety Champion sends details of initiative plus evidence that it is in place and working; 2. I score inititiave according to: - i. how many people benefit; ii. the hierachy of controls i.e. removal of a hazard gets more points than providing PPE; and iii. how original the idea is. 3. Score is fed back to the originator allowing time for an appeal of the decision; and 4. Points added to running total on Health & Safety Monitor. I didn't offer any prizes but I suspect that prizes were given by local managers according to how well their Champions performed. For me, the competition which developed between sites / departments etc. was a shot-in-the-arm for the health and safety effort over all. I'm not sure what else I can tell you.
