hi,

company policies notwithstanding - would a sparkie who's been through 17th edition be expected to know that distribution panels should be isolated whilst installing a new MCB? Is this covered in 17th edition?

If memory serves me correctly there are occasions when live works are neccessary and there are extra controls that can be used to manage the risk. It would depend on the nature of the works.

I would think live work only for testing not during an installation??

In this case - the IP accidentaly connected 2 phases and suffered from an arc blast.

There was nothing being run from DB panel and this was an additional installation. Company policy states that where possible all works on DB panels should be carried-out out of production time and power supply isolated. The input was not isolated.

Sounds like it was more likely to be complacency then.

Nothing like learning the danger of complacency the hard way!

with modern technology its very rare that live work in an average work situation needs to be undertaken - my advice is to return to the RA and ensure that things are managed as they should be from there on in

The last man that I managed personally re electricial working was dead only a few hours after he had been inducted even though all work on that site was via PTW and electrical work carried with it an aditional PTW - he had wanted to impress and got stuck in before his colleague had returned from opening the Electrical PTW for that area

The night that I had to see his wife was one I never want to repeat so again I say do NOT work live unless there is no other way irrespective of other pressures and only work if you have all safety and emergency systems in place - systems that have recently been tested not systems that have been in a draw for years
