At what height above ground level does a hoop arrangement have to be fitted. I have been told varying degree's of height ranging from 2m - 3.5m, so thought I would pose the question to a more competent group.

I have already taken other other fall arrest systems into account so please direct your answers to the specific question.

There is no prescribed height where hoops must be fitted to a fixed ladder
Hoops on a fixed ladder are not a fall arrest system.

Hi Elfy,
The height should be 2.5m above floor / ground level (BS 5395 Part 3: Stairs, ladders and Walkways - Code of Practice for the design of industrial type stairs, permanent ladders and Walkways).

Dave, thank you I was aware that hoops / cages etc are not a true form of fall arrest, there must have been a block from my mind to my fingers!

I have also been told between 2.2m -3.0m from ground level for safety cages / hoops. This is contained in BS4211:2005 Permanently fixed ladders.
