Hi, I'm just reading through an audit carried out on our H&S System last year tying up a few loose ends. Under Accident recording it states:- "We must ensure our Accident Book entries are cross referenced"..... i.e. the number filled in on the top of the torn off section is copied in on the section that remains near the spine of the accident book. Just wondering why this is necessary. Any Ideas ?

If anybody claims that you have "disappeared" any of your accident reports or investigations for your "benefit", if you have your documents well cross referenced, it would be easy to show otherwise. even accident forms that have been damaged during the writing of the report should be kept.
