Record of maintenance of a lifting appliance

I'm trying to confirm whether an employer must have a record of maintenance of a lifting appliance carried out. This used to be documented on a form called the F91. You could get this form from Her Majesty's Stationary Office but this was removed some years ago.
The record of inspection was carried out by the operator every 7days and documented using a form such as a F91.
I've examined LOLER and under reg9 it states: "Weekly inspections of lifting machinery will depend on the equipment and where and how it is used but could include, for a crane as an example, the
correct operation of limiters and indicators, checking tyre pressures (if mobile equipment), checking that no components are missing, eg bolts, and that the controls work properly. Further recommendations on weekly inspections and daily checks for cranes are given in BS 7121."
I've looked at BS7121 for guidance but this standard is designed specifically for cranes and lorry loaders and not lifting appliances.
Can anybody please clarify if the employer has to carry out weekly inspections of lifting appliances and are they to be documented and if so, how?
