We have guys who install fencing and prior to any digging works, such as creating the holes to lay fence posts, all areas are subject to CAT scanning.
I have now been asked about our flaggers, if they have to dig out the top soil etc at what point do we insist that they check for underground services? We do have instances where flags are simply lifted, the ground re-levelled and flags laid back down, equally we have situations where drives are flagged and therefore the ground needs to be dug out and hardcore laid prior to the flags etc.
I do not want to go down the route of overkill, but equally am unaware of an industry standard or guidance that says this is best practice etc?
Any help is gratefully appreciated.

Risk assessment.
If replacing existing with the same then probably no need. New drive then probably need to scan.
The power to my house comes from next door's garage? Funny set up but it runs under the front garden to a position where my garage used to be. (Since converted to bedroom that's how I know).
I contacted the electric company who had a drawing and notified me so I could inform the company that changed my meter.
If I had not got that info, and didn't bother to cat scan either there would defo have been a problem.

Unless you have x-ray vision I would suggest that before you penetrate ANY external ground with any item you CAT SCAN the area –over the top?

My experience is you need service drawings. But don’t just rely on them; they should only be a guide. All too often the services are not where the drawing shows them to be.
It’s best to use a CAT and if possible a GENNY, if you find services you should hand dig and re-scan ever 150mm until you reach the depth you want to be at or you uncover the services.
