Is anyone aware of a recent incident involving a UK hospital patient being found deceased under a toppled oxygen cylinder? Apparently this was a large (G sized) cylinder. This has been the subject of an NHS Estates alert but I am surprised not to have seen it mentioned in any journals such as SHP or the online resources. Maybe I have just missed it?

Tobin - maybe as this originated from our colleagues in NHS Scotland that this was an incident in their neck of the woods. Like you I certainly don not recall seeing any reference to this case anywhere else other than in the NHS Estates alert.

Almost all of the mainstream safety magazines/journals tend to report prosecutions or coroners reports rather than safety alerts that are issued by multitude of organisations, therefore I am not surprised that a DH Alert dated 28 July 2010 regarding "Unsecured medical gas cylinders, including cylinders on trolleys" has not been covered.
