Hi, I'm tasked with implementing a new Permit to Work system at our site which is a food manufacturing factory. I'm totally swamped and already developing and implementing asbestos procedures/mgt plan, fire risk assessment, training and improvements, a contractor management system and procedures, PPE policy and writing budgets for my dept and other depts (h&s requirements) and i really do not have the time. I have however just had a near miss event that requires the revamp of the PTW system asap and i can't just place this too the back burner. Our current activities requiring PTW would be electrical works, hot working, confined space, breaking into product, chemical and steam lines, working at heights. At present the maintenance manager ships out responsibilty of his contractors conducting maintenance work to dept managers to complete the PTWs on the grounds he does not know what the production managers have in the system (CIP wash, product, etc). My experience of PTW have always been all mantainence dept to sign of as 90% of work is theirs or their contractors. Any help would be great. (stressed out safetman)

Add the department managers sign off on to the form and to complete any relevant information like CIP washes, products etc. PTW should be signed and agreed by all relevant persons affected by the work but someone needs to be in control of the system and what is or isn't taking place which would normally be a senior manager in the maintenance department and should be involved in the process. If someone has important information to the work then they should be included in the PTW. It's no good the maintenance manager taking responsibility for equipment he does not know the condition off, you could argue he could find out and then sign off put that isn't using proper control measures so i would look to the manager of the relevant department to sign as well (who as control of that equipment) But they need to be trained and understand what work is involved rather than signing without understanding the consequences.
