Does anyone know of any cases where individuals or companies have been successfully prosecuted for failure to PAT test electrical items and indivividuals have been killed or seriously injured as a result of their neglect?

its never gonna happen! Since PA Testing isnt a legal requirement then "failure to PA" would never be a charge raised.
If anything ever went wrong with electrical appliances, and led to a serious incident, then "failure to ensure electrical safety" under the Electrical Safety Regs, or failure to ensure a "safe place of work" under the MHSWR Regs may apply.
Try searching for "electrical safety prosecutions" etc

You might already know this but it is not a legal requirement to pat test electrical equipment but you are required to maintain it and have proof of this. So for some companies it's easier to appoint a contractor for other it's easier to visually inspect themselves.
i would have thought it unlikely anyone would be prosecuted for failing to pat test it would have to result from injury or death as you say. I'm sure there have been injuries or death (certainly) but more likely with domestic appliances in the home.
