I am trying to find out details of a case a few years ago where a Health and Safety Consultant was fined £11000 for an inadequate risk assessment on a Spindle Moulding machine
Does anyone have any info on this links etc
The case was very high profile at the time

Isn't it scary that someone who had been the chair of BSC and another safety council could make such a grave error and then also to try and get outn of it by saying he wasn't familiar with that type of machine - well he shouldn't have agreed to assess it then!!! Are people like that really setting the standards in our safety councils?????
It's scary that some people don't know where their competence begins and ends. I know I'm one of the first to ask everyone not to tar all consultants with the same brush but there are some incompetent fools out there.
It would be good if the HSE published this and other examples of bad assessments, just to show others what will not be accepted (10 to 1 says the risk assessment just said something about providing guards but not what type etc)

Mr Hooper was not the Chair of BSC, but the SHPOarticle reports that he was a board member from 1993 to 2002 and chair of the Southampton Occupational Safety Association between 1998 and 2003.
The was a significant change regarding the way BSC was governed after the passing away of James Tye, but it did take some time for the board of governers to be replaced by a more robust team.
