I am interested in dumper truck accidents that have happened in the UK. This is following a fatal accident with a 9t Barford that I have been involved with. If any safety officers have been directly involved please could you contact me by e-mail [details, make of dumper, basic causes, extent of injuries, if photies are available etc]. thanks for reading this post.

having delt with many over the years the man causes are, in my opinion; *lack of management responsibility, interest, compliance monitoring, enforcement of the rules etc *lack of training *On CDM jobs; lack of time to get the work done *Absence cover where there is not enough staff so anybody takes over to get the job done *Some really good risk assessments and WSSW [method statements] but no adhereance to same & in many cases the day to day staff have not seen the documents nor been informed about them * Poor maintence - noting that for kit to fail the maintenance has to be bad over a long period I have only delt with 1 manufacturing failure in >20 years and that was a towing situation not a role over situation Hope that this info helps

Bob Confidential - that suggests that the cases you refer to have not yet been heard in court ie where evidence is put into the public domain. Steve

Steve HSE prosecution database should help (ditto prosecutions history database) http://www.hse.gov.uk/pr...V=dump&x=24&y=12 Many years ago I investigated accident involving overturning 16te dumper. Driver made the mistake of trying to jump out and had leg severely crushed. One of 6 brakes missing, thence affecting steering. Contractor fined £800 against then maximum penalty on summary conviction of £2000. Regards, Peter

Here's another: http://www.hse.gov.uk/press/2010/coi-sw-ww833.htm Probably the contractor will be exempt from any sort of risk assessment eventually. I think I see some problems !

If my memory serves me correctly a major contractor whose name begins with C has banned 9 T dumpers from their site due to their height. In essence the driver cannot see some directly in front of the dumper. It might be worth contacting them for further details. regards Yul

Yul Thanks, I'll be contacting them. I am looking for other evidence in the form of accidents that may not have been reported to the HSE, that Barford 9t swivel is involved in overturning incidents and/or has particular stability problems.

Steve this is not exactly what you asked for but should be of interest to you with your information gathering. This is a research project carried by the HSE in 2008 in conjunction with MIRA on swivel powered dumper trucks and causes of roll over. The Barford was not used in the trials but the still relevant to your investigation. http://www.hse.gov.uk/research/rrpdf/rr614.pdf. Steve

Steve, I have a classic dumper accident I was involved with last year involving a Barford 3 ton swivel which has now been fully investigated by HSE. I have some amazing photos which I am happy to share as a lessons learned exercise obviously keeping them anonymous to protect any parties. Bottom line, brand new dumper in excellent condition, good ground conditions, suitable and sufficient RA / MS, driver experienced and reassessed NVQ only 3 months previous with comprehensive video evidence of assessment. Injuries caused by not wearing seat belt and raising bucket whilst possibly still moving. You can take a horse to water but when your backs turned!!!. If you give me your contact details on reply I will forward some photos when I get to work tomorrow. Hope that helps. Regards Andy M

Hi Andy M, I would be gratefull for a copy of your photos etc as well if you wouldnt mind please. I am here in the MENA region where standards are terrible generally-huge need for education here. My email is-calum.cameron@hotmail.com I actually went to look at a dumper roll over a few years back and found it was purely down to in-appropriate use by the driver who drive across a bank of spoil diagonally. Luckily it was caught partially on CCTV so i have it on video. Regards Calum Cameron CMIOSH

For background reading you could take a look at the R v Chargot case that involved a dumper truck fatality. Just had a quick search for details and none of the numerous sources seem to mention the type of dumper unfortunately.

Stevie, Theres one on the IOSH home today regarding a 14 year old boy being injured on a construction site, however i think the response by the HSE Inspector is worrying as my first thought would have been that he shouldn't even have been on the site in the first place. Regards, JJ

Many thanks. I think the inspector did right prosecuting for a 14 year old driving a dumper. I think more charges should have been put forward. This has got to be one of the most dangerous items of plant in construction and they let a child drive it. Crackers.
