Our current Occupational Health Questionnaire as I believe a lot of questionnaires out there not taken into account the Equality Act 2010

I need to change my questionnaire to fall in line with regs

Can anyone pm me with a current copy of there that I may modify to suit for myself that takes into account the regs

As your question raises many far-reaching HR issues, it is advisable to check with the Equality and Human Rights Commission rather than approach adaptation simply as an OSH matter

Alan,

Why not discuss with your Occ Health provider? They may well have a questionnaire that covers this.

In addition any areas thrown up should be referred for advice anyway?

Also is the EA 2010 mainly referring to pre employment and some questions can be asked if it's integral to the position etc?

I have asked for advise from Equality and Human Rights Commission and would agree this would be ideal however they have stated they will only get back in 5 days which puts me in the mire as I have two new starters arriving on Monday

1 receptionist and 1 labourer both different questions required
