Good Morning all
We are setting up a new site and have just got scissor lifts delivered, however I want to run as is required familiarisation training on this kit
I was looking for a form to be signed rather than a toolbox talk form
Has anyone got one I could make use of and adapt to suit
Thanks
Alan

Have any of those who will be receiving training have previous training on scissor lifts before, are their certificates still in date. if yes then familiarization training sounds like a good idea (who will be giving it) as for sign of sheet just produce a table with a title of the course a list of names which they can sign against the date and who ever provided the familiarization to say they have all received the training and are competent.

All operatives are IPAF trained and have current certificates.
I will be giving them familiarisation training by taking them too each machine letting them bring their machine down to a level using the base controls
I have four machines on site so wanted to ensure they signed for all four, as you say may just use 4 TBT with machines identified on each

I think it wil;l al depend how your compnay keeps track of records as they may want a sign of sheet for each individual so they can keep it in their personnel records or a group sign of may be fine, tou could just list all the equipment numbers in the title if they are are all going to be shown all 4 or if they are basically the same machine.
