Hi all,

My name is Kirk I am in my final year of an MSc in RCDM at Leicester UNI. I am currently writing an essay based on anonymous reporting systems discussing the proposition that they guarantee active learning.

I am interested in any ideas or thoughts members may have.

Kirk, may I suggest you contact the Rail Safety Standards Board. They operate a distinctly separate system called CIRAS for confidential reporting of safety related incidents within the rail industry.

You could try the RAF (good luck). They had and may still have, confidential reporting systems for both aircrew (CONDOR) and groundcrew (MURPHY). The forms were widely available' often on the back of toilet doors (handy if you just had a near miss with another tornado!). Some of the reports were subsequently 'shared' through the RAF Air Clues magazine (now defunct I believe) as a learning opportunity for all. Mostly very interesting reading.

Brilliant Phil, toilet doors and near misses !! I know the Royal Navy fleet air arm have one, sounds similar I'll check it out thanks.

Peter thanks also some good info, the NHS (NPLS) look a bit of a monster but very interesting.
