I have just found this from a UK trade association.
Glutaraldehyde is allowed for use in cosmetics up to a maximum safe concentration of 0.1%. Where present, it will be labelled as ‘glutaral’ in the ingredients list and the product must also be labelled with the words “contains glutaraldehyde” if it is present above 0.05%.

I am personally not aware of an outright ban, but it is an asthmagen and in most cases its use as an disinfectant in healthcare settings has been "susbtituted" by safer disinfectants.

Well you can buy it from Sigma Aldrich chemical company, so doubt it has been banned. It is normally packaged as a an aqueous solution. It has been linked with occupational asthma with people working in the health care profession, where it has been used as a disinfectant.

I can't find anything specific about any outright ban so its unlikely but as always CosHH - eliminate or substitute. Bans tend to be either general with exceptions or relate to a particular process (i.e. as found in schedule 2 to CoSHH).
Some organisations may ban a substance . There was a prosecution last year http://www.hse.gov.uk/asthma/glutaraldehyde.htm - The NHS no longer uses it.
Marty, even if there was ban it may still be available through the likes of Sigma for research / testing purposes (e.g. testing for levels in cosmetics). Control of supply of these may be tighter than general 'lab' reagents though.
