Can any one help me how to prepare environmental assessment & impact register?

Think of them as an environmental risk assessment.
Firstly identify all of your environmental aspects - processes that may have an impact on the environment. Things like energy use, the use of packaging, transport of goods, processes that might result in emissions that could have a polluting effect, storage of materials etc. A sort of environmental HAZARD
You then look at what the possible environmental impact would be of each process. A sort of environmental CONSEQUENCE.
Some organisations group them in the following categories:
Emissions to air
Emissions to water
Natural resource depletion
Emissions to ground
Waste disposal
Nuisance - Noise etc
