Hi all,

last roll of the dice here.  Years ago, there was a video of a gentleman giving a heart felt safety presentation which started off talking about his kids and led on to talk about a story where he was visiting an oil platform.  The story played out where there was a tool box randomly sitting on the hele-pad (I think) and he was subsequently killed.  He continued to tell the story of his family being told the horrible news etc.

For ther life of me I cannot even remember some simple details to enable me to serch for it.  It sticks in my mind the guy was working for (or presenting for) a company with Wood in the the title, but I'm lost trying to find it. 

Any assistance is greatly appreciated.  Fantastic video to drive home dealing with hazards when you see it.
