I am using Croner to record accidents/incidents but would like to work up something for our staff to better report accidents/incidents through, rather than via paper or email. Does anyone have any systems they are using or systems they have worked up?

We had our IT department build a system for us which is ever evolving. We issue every employee with a user name and password and the system is available on 99% of terminals around the site. Our reporting of near misses has gone up around 150% since we implimented the system. Accident reports are done virtually straight away and the relevant people informed straight away. I am happy to discuss the system if you wish and our IT department is a stand alone company so they would be happy to speak to you about it. Years ago, me and an IT colleague knocked up an incident reporting system using Access. It was exactly what I wanted-not pretty but functional and it particular it created data that I could analyse.  Nowadays I suppose that are other packages that would enable you to create such a system from scratch. The problem with a bought in system, is that you often end up having to modify your procedures etc to accommodate what the system wants, rather than the other way around.  Think about what it is that you want it to do and how you want to use it before deciding what to go for. Don’t let the vendor tell you what you need to be doing.
