Good Morning, thank you for your reply, thats the spirit that we need show in the health and safety community, it was not blatant and was just a question, if I could help anyone concerning the health of others I would, with out the need for a monetary reward, thank you

Such a template is quite an extensive piece of work. Not just a quick simple record type form which can be put together in a few minutes.
Requesting such a form suggests you are going to do an L8 assessment yourself - in which case why not develop your own template.
Are you a consultant? Are going to use the template multiple times in the future - hence make money based on someone elese's work, which you have obtained for nothing?
A consultant/HSE adviser can't protect assessment forms/templates once issued and used - but it is totally unreasonable to blatantly request such extensive documents.
