I have been shown a purchased RAMs where the publisher uses Severity categories F M R S M. I am puzzled by the R and S in this context.
Thinking S may mean 'significant' or 'serious'? Any thoughts please on this and what R may stand for? Thank you.

I would guess "reportable", "recordable" or "RIDDOR".

If there is no key to decipher the document to render it usable then it is not worth the paper it is written on and should be rejected. "We" should not be speculating on what the provider meant - it is them from whom you should be seeking clarity.

First aid, Minor, Reportable/Recordable, Serious, Major are often used as labels in this context. Normally though you do expect them to be spelt out.
