Good morning all
I wish to gauge opinion and what actions you believe should be taken for this scenario: A forklift truck driver inside a factory had to stop operations due to a hydraulic hose becoming detached and spraying oil out about 20 feet onto the shop floor and onto an employees work boiler suit overalls - it did not impact his skin but resulted in him having to change his overall. The oil was quickly cleared up using spill dry and packaged correctly.

i belive all incidents should be investigated, the depth of the investigation will depend on the realistic consequences, but i would want to know why the hose became ditached - (lack of mainatance etc) so that approriate actions can be taken to try an dprevent a recurrance.

Well of course - the reason being to find out how to prevent recurrence which could easily result in more serious consequences than on this occasion.

I would certainly be ensuring the incident is investigated due to the potential for a much more serious outcome. What if a person was standing at a much closer range and the hydraulic fluid entered the eyes at pressure? You need to understand why this happened eg. lack of maintenance, poor maintenance procedures, damage or something else to prevent a reoccurence. I'd want the FLT examined by a competent fitter before further use to ensure that no defect was apparent.

I would certainly investigate this, as others have rightly stated. A hydraulic hose does not just break for no reason. Things to consider during investigation - Is the operative trained to a competent level? Are pre shift checks carried out at all and if so is it just a box ticking exercise? How robust is your reporting process regarding damage to FLTs? How effective is your service contract wth FLT maintenance company? Is there a company policy that defines what must be done, when, where and how regarding MHE in general?
A good indicator of is the overall condition of the truck and if it looks like it has been through the ringer then it points inadequecies in the safety culture either with the individual, department or organisation as a whole. Food for thought. Good luck.

Hi, good question, and I have known this to occur with both oil hoses and propane hoses. This should be recorded (even as a near miss or property damage) as the medium/material sprayed could of hit eyes, or worse punctured through skin (which needs immediate hospitalisation and cleaning due to likely gangrene and major infections (trust me on this).
Like the other person said on here 'Why did it break - maintenance issue, overpressurisation, unexpected wear & tear or damage?'.
