An FLT Operator on my Site was moving several slab like test weights in to their storage location. Whilst moving it the 600kg slab detached from the 4 x lifting rings and dropped to the floor from travel height. The lifting rings have bolts which then are threaded in to the test weights, the thread is inside of the test weights. It is clear that although the lifting rings and their bolts are within their 6 monthly inspection, the condition of the bolts are poor. The embedded thread is also of poor condition. After looking at the RIDDOR reporting criteria, we have deemed that it does not fit the criteria as it is a lifting accessory which failed. My only thoughts on this are surely the failing of lifting accessories can be a dangerous occurrence? I.E sling(s) failing on a heavy lift or other examples. Another thought - I am unsure whether the threads should receive a LOLER Inspection as well? as they form part of the weight bearing component when the bolts are inserted.

Hi Andrew Lots of things that can happen which you and I may think should count as "dangerous occurrences" but are not within the scope of what is reportable under RIDDOR. I think it is fairly obvious that if the condition of the threads is critical to maintaining a safe workplace then you need a system to check their condition. May be it's a requirement of PUWER rather than LOLER or simply implicitly required by the parent of both codes of regulations, i.e. Health and Safety at Work etc Act 1974 Might be that other things about the scenario are such that it is not reasonably foreseeable that someone will get hurt if this test load falls. At that point it is a business loss issue. How much property damage is going to happen. Do you want to prevent that?

It's an occurrence and it's dangerous - that doesn't make it a reportable dangerous occurrence. And not being a reportable dangerous occurrence doesn't mean it didn't happen or isn't dangerous. So I agree with Peter - the fixing devices clearly need regular inspection.
Once when I started at a company I was shocked to see a whole shelf of ring binders labelled "Dangerous Occurrences". Before I could run away from the job, the person showing me round said "Don't worry, that doesn't mean what you think it does."
