I seem to have been blocked from logging into my chrome extension (even though I did log in a couple of days ago). How can I re-set this?

There is no Chrome extension for iosh - do you mean your browser bookmark?
You need to be careful when you actually bookmark a page, do it on the log-in and next time you follow the link the url will be unresolvable (which sounds like your issue). Follow the froum link from the iosh home page http://forum.iosh.co.uk/ and bookmark Then you should be able to login following the link on the top right
