Hi all, I have a general H&S audit template which covers a number of questions under 4 headed sections, what I am looking to do is to create a scoring matrix for each section or question. I'm struggling as to what would be the best to use as this would need to be easily understandable to the managers of the area being audited. This is a general audit and not to 45001 Has anyone got anything I could use or suggest/recommend anything?

I think you are creating a rod for your own back! Why do you need to score the audit?

The audit should simply tell people if something is ok or needs improvement. Any numbers that you assign to things just make it look “scientific” without actually giving you any useful information.

It needn't be as complicated as a matrix.

Each answer can be rated as OK / Not OK (or other terminology to that effect).

Then just add up how many are OK.

SPR - as soon as you introduce a scoring system you are likely to introduce skewed results unless you further complicate things by adding some form of weighting.

So, suppose you were auditing against compliance with the CDM Regulations on a construction project.

One question would almost certainly be

"Has the project been notified to the regulator on Form 10 or equivalent?" If your scoring system is say 0-5, it's very easy to get a 0 (but also very easy to close out the action) or easy to get a 5.

If you then ask about what the designers have done to comply with their general and specific duties it would be very difficult to score either 0 or 5 (IFF the auditor looks closely). But it should take much more effort to move a score from 1 to 2, or 3 to 4. 

If you have in total say 10 questions it would be quite easy to get a headline score of over 90% and mask a significant non-compliance on one issue and even easier if your questions are looking for deliverables rather the quality of deliverables.

Even a Form 10 could vary in quality!!

But CDM requires a number of deliverables. If you are doing a tick box exercise then it is very easy for a scored report to overlook that some of the QQ are MUCH more important than others UNLESS you include some weighting system.

AND whether or not you include weighting, people soon learn how to play numbers games (whether or not intentionally).

I would go further than Kate. A single "Not OK" means you have a problem, even if the other nine QQ have "OK" as the result.

How is the audit being rated at the moment. Some show the element and outcome then rate I pass/ minor issue / major issue.   Rather than a number, could you show it as 20 of 31 elements pass, 4 of 31 minor and 7 of 31 major.   Rather than a score, you have an immediate visual of the outcome. using number can creat confusion when you are not sure if a high number is good or bad.

Hi,

Mine is percentage based scoring system. Pretty much you get 1 point for yes and 0 for no (i'm a view of you do it or you dont). That turns into a percentage score and each section will then produce a total (using an average formula).

Just for fun it will also give you an overal scoring for all sections in the forms of stars (5 Stars is 100% for everything).

mines has alweays been based on risk which then equates to how quickly any action should be completed.
