Hi all

Can anyone remember a case (forgive the vagueness) that I saw on a documentary a few years ago  where a staff member was working in testing facility with explosives, they were supposed to set off the explosive and wait a period of time before re-entering but as nothing had happened for years she went in early and was killed, the presenter discussed how the accident was put down to complacency of the role.  Im thinking it was somewhere like Aldermaston or Porton Down (though Im probably wrong)

Again sorry for vagueness, google doesnt help and its been bugging me for years!

As someone who used to work in  Aldermaston it wasn't there. I'm pretty sure it was Porton down, but thought it was a chap a couple of years ago? Explosion at Salisbury military factory leaves one dead | The Independent | The Independent was it this?
