Squire - what do you actually want to achieve that a simple spreadsheet or similar wouldn't deal with? Plenty of offerings on the market. Whether they suit your requirements is an entirely different matter. A lot of this will come down to how much data you might have (either currently or with better reporting) and what you wish and need to do with it.
So, as example, you might need it to distinguish between different legal reporting requirements to national authorities. So, you might need to distinguish between (a) incidents that require to be notified to the HSE in GB within specified timescales. (b) incidents in GB which do NOT need to be reported to the HSE but DO need to be recorded should the regulator or someone else (e.g. an ISO auditor or client) then come knocking on your door (c) all the stuff that you need to collate for events in the US and then need to report in summary format on an annual basis to OSHA (d) and lots of other variants You might wish it to provide comparison of different global locations using the same parameters for e.g. severity of incident. You might wish a handheld system for electronic communication of all "near misses" or whatever terminology you use for reporting of dangerous scenarios.
