Stephen - i think the problem is we all use different systems, a lot will depend on type of vehicle you are using, where you are using it (public roads, private land etc) and what you class as an incident - (eg near miss on road, overturn, lost load etc). I had one of my collegues who is good with excell in a previouse create us one with the options we though we needed - and then added the ones we had forgptten as they occured. was not able to find one online that did what i wanted.
