Afternoon all, I'm specifically looking for lessons learned notices, bulletins or similar, following on from serious or fatal accidents. I know the HSE has their safety alerts page, but this doesn’t cover particular cases. With all the investigation work that goes into these cases, it seems illogical that there isn’t some kind of lessons learned information for other businesses. Otherwise, we will just keep making the same mistakes? The reason I have flagged this is that there has been a recent fatal fall in my sector involving the same type of construction work my client does. It got me thinking about this… Cheers

Hi AB About this time 21 years ago I did a presentation at IOSH National Conference "Sharing lessons learned from accidents" as the "Douglas Short Memorial Lecture" which used to be part of the IOSH Construction Group annual calendar. Most of the presentation veered to the opposite of the title as I was commenting on the inability of the sector to share lessons learned. But I did pick on some notable exceptions, including a little spate of incidents involving semi-automatic hitches on excavators used for lifting. The HSE had not picked this up but what was then the Major Contractors' Group highlighted it to HSE who then put out an announcement. The picture since has been mixed. There is still a reluctance to tell one's competitors about things that have gone wrong (partly as more often than not it means that you have made a mistake), but some organisations do get together and help each other learn. In construction, the best sources are probably the CROSS and SCOSS websites. But, if you do your networking, you will get into sources of other "alerts", some of which show that effective root cause analysis has clearly not happened and sometimes the alerts send dangerous messages. In the early days of the pandemic I saw more than one Safety Alert to warn about the supposedly significant fire and explosion risks of alcohol based sanitiser. Didn't take long to work out that these alerts were poorly informed - but, of course, once out, quite difficult to counter.

If you really want to share lessons learned what you need to do is set up a group of like minded people, who are willing to share their H&S experiences. You need to be like minded in the sense that you are all trying to genuinely share stuff and you have a similar level of risk appetite. You want to share genuine lessons but avoid the OTT responses like Peter mentioned.

Network Rail circulate bulletins among the industry of lessons learned from rail incidents. It's a good format that could be adopted (but I don't know if other sectors have).

It depends which industry you are looking for..if you are only looking for Safety Alert types, there are alot, i would not call it lessons learned..it is only information about what has happend But if you are really want to know what has happend and what safe guards have been failed in organisation that led to that particular incident with are required details (I call this one lesson learned) there are not many ..
