Hello I am looking for recomendations or useful pointers in the best type of accident reporting systems please. We have online one using MS Forms but really need to look at something that assigns actions and workflows to managers following the incident/accident. We're trying to move to a place where managers are responsible for managing these with an overview from H&S Team (I appreciate that is a whole different discussion). In previous life I have used the Datix system in the NHS and this would email managers with actions from the incident.
It doesn't have to be web based but ideally like to be able to have the chance to format to our needs. I have looked at some off the shelf systems but need the ability to make specific to our requirements and not costing the earth. (I may be asking for the world here though!) Would anyone be able to point me in the right direction of suitable systems or contacts please?

Hi Lishka0 A variant of a question asked many times on these Forums. You probably have some IT whizz kids in your organisation who could put together what you need. What they need to know is what you want to achieve and then they can work out a programme to deliver what is required. Then you get something bespoke to your organisation and its specific needs!

The best system I ever used was put together by an IT whiz using a Microsoft access database. I told him what we wanted, and he delivered it 2 weeks later. By contrast working with a vendor will involve lots of money and time and questions about how to transfer data from the existing system to the new system. all singing all dancing systems are rarely of Broadway quality, more like the local opera society Gilbert and Sullivan- the poster looks good but the end result is a bit disappointing.

MS Forms -> SharePoint List -> Power Automate to generate emails and reminders. You can even intergrate with Power Apps.
The other option is to use IAuditor from Safety Culture. Its free and allows for actions to be assigned with work flows. Not sure if its compatable with forms tho

MrBrightside Thank you - thats what we are trying to work with at the moment. Have also been told about IAuditor so that one is on the list as an fallback.
A Kurdziel - I agree, we have explored many options and looked at developing internally but its coming up with some challenges. I would assume working with a company would also have its challenges and may not be as be-spoke as we would like it. Broadway quality is a phrase I am banking for later use thank you :) although I would settle for Gilbert and Sullivan
peter gotch - Thank you. Our IT whizzers are trying to help get us something specific with Forms and Power Automate so sounds like this will be the best option. We may get that West End version rather than Broadway, who knows!

Hi, I use a company called ehasoft, they cover audits, corrective actions, accidents/incidents, near miss, r/a etc, yu can assign actions with close out dates etc etc.. really good system.
