Good Morning. According to our new SHEQ that we check in-house our fire extinguishers, for example, on a monthly basis using a form which is dated and signed. we also are required to produce a certificate as well! Bearing in mind we outsource full annual checks. Is this certificate required when already date and sign?

I don't understand what this 'certificate' is. Who is certifying what, and on what basis?
All you need in terms of ISO 45001 is 'documented information', which can be a form you fill in or any other format of 'documented information' that suits the purpose.
However if your management system says that you produce a certificate, then on the ISO principle that you have to do what you say you do, you will need to produce a certificate (or else change what the management system says).

You keep it simple - our system states an external provider appears twice a year (due to the number of extinguishers) and provides a report at each visit.
That report is actioned and filed in evidence that inspection and maintenance is happening.
We don't even have paper any more the report being a pdf attachment to the suppliers electronic invoice.

Is this certificate being reffered to the paperwork issued by the company servicing the extinguishers? I can't speak to our office manager currently as she deals with it. But apart from the sticker on each extinguisher, does your service company provide any further paperwork?
