Hi MJ - seems to me that what is far more important than a template for an asbestos management plan is whether the survey reports that you have stand up to scrutiny.
[I saw my first absolutely rubbish asbestos survey report circa 43 years ago and have seen many rubbish asbestos survey reports since, even when I have drafted the specification to make clear what would NOT be acceptable]. If they do (that's a very big IF) then surely all you need is: 1. Location 2. Type of survey - management or demolition/refurbishment - in your case the latter is assumed with all that goes with making assumptions! 3. What has been found - No Asbestos Detected (NAD), Asbestos detected and its nature, Asbestos presumed, Absence of asbestos presumed ....with in each case a summary of the condition of the material + whatever caveats have been written into the survey report - which gets you back to questioning the adquacy of the survey 4. What you are going to do about the detected or presumed ACMs, who is going to do what and when. 5. What inspection routine and/or further survey work may be needed in the future and when. So you have e.g. a large building but have only surveyed part of it in advance of some refurbishment work. You may or may not have commissioned management survey(s) of the other parts of the building - you need a clear idea of what has been fully, INTRUSIVELY, surveyed and what has not. Doesn't need an OSH specialist to put together a template for this. Just anyone with the right basic techie skills. If you tell them what you want to portray and give them the information to populate the template, the rest is easy for someone with those techie skills.
