I might be missing something here but shouldn't you provide him a harness for his weight? PPE for the person and not a person to fit the PPE? If this cannot be provided surley he should not be doing work that requires the harness? Again, appologies if I have misread this.

To which I'll add that I don't understand what it is that is to be "documented" (that there is a weight limit? that this person is excluded from the work? that this person is going to do the work anyway?) or why documenting it (rather than solving the problem) is the concern. So I am definitely missing something.
