Posted By George Wedgwood I am researching instances of safety management systems failure where companies have been carrying through some kind of change programme to reshape their business or downsize to minimise costs. These can be related generically but I need 4 or 5 cases for discussion with managers, to emphasise that attention to the details of H&S management at such a time is vital for company continued excellent performance. If you have any experience or examples of such failures that contributed to enforcement action or intervention by a Regulator, or even resulted in a catastrophic loss, I would be very interested to hear about them. You might also try searching the HSe's site with "organisational change" as your search term - there's particular at the HSE about its effects on nuclear and railway safety.
