I'd be grateful if someone could point me in the direction of a good risk assessment form for work equipment, particularly covering more comlicated machinery / guarding. I have a copy of the form prepared by SAFed, but wondered if there are any other sources available. Many Thanks.

If you plan to study this issue in depth, then obtain:

Safe use of Machinery, PD5304:2000
Safety of Machinery -Principles for risk assessment, BS EN 1050:1997.

I have devised a system which uses a preliminary hazardous sheet based on BSEN 1050, which then focusses attention to specific risks caused by the work equipment. A risk assessment is then undertaken in the normal way based upon those hazards and remedial action taken. If you would like to e-mail me I can send you copies of our system, which you can adopt to suit your needs.
