See the recent 'What's New' posting on the HSE website of a research report evaluating use of the HSE tool. This includes detailed reviews with some users re what they learned from using the tool.

There is also an area in the offshore industry 'Step Change' website (link via the Offshore Specialist Group from the LHS of this page) devoted to the 'Behavioural Issues' Work Group which has some useful information. One of the key references developed via Step Change is the report "Changing Minds" which has loads of supporting references and some good tools for use at different stages of cultual maturity. The 'Safety Culture Maturity Model' (Appendix 1 is one of the most helpful assessment tools I have found.

You may be interested in a very good article in the December SHP entitled 'Don't shoot the messenger,' although not directly linked to safety culture, it gives an interesting view on 'blame culture.'
