I have been tasked to produce a workplace assessment form for use in the NHS.In particular, looking for an innovative up to date grading system.
Does anyone know if there is a generic form online which could be altered to suit?
Any help would be most appreciated.

Have you looked at "Safecode" which was specifically developed for the NHS. Run by a company based at Strathclyde University.
