Posted By Robert K Lewis The problem with 5x5 matrices and larger is that it is difficult to shade the assessment sufficiently clearly - this tends to decrease the uniformity of assessments. I still prefer the 3x3 splitting into 5 categories, see HB 10180 - Managing Safety The Systems Way, page 48 in my edition, - available from BSI. E-mail me if you can't get a copy to view the matrix there. It is a very useful document even if you are not intending any form of certification. Bob
