Hi
I am writing a risk procedure and require some assistance on putting together a 5x5 risk matrix.....How to define the consequences 1 - 5 and probability 1 - 5. Its to cover employees, public and environmental affects of the hazards
Any input would be greatly appreciated

The problem with 5x5 matrices and larger is that it is difficult to shade the assessment sufficiently clearly - this tends to decrease the uniformity of assessments. I still prefer the 3x3 splitting into 5 categories, see HB 10180 - Managing Safety The Systems Way, page 48 in my edition, - available from BSI.
E-mail me if you can't get a copy to view the matrix there. It is a very useful document even if you are not intending any form of certification.
