Don't laugh, there is a lesson in there somewhere

I'm posting this just because I have to tell someone !
I have just returned from visiting a warehouse. While there I looked through some recent accident inquiry reports and found the following :
An employee was loading sacks of product into the back of a lorry. Because of a physical obstruction (railway lines in front of the loading dock) the lorry could not back up completely to the dock. A large plank was used to bridge the gap. This had been normal practice since they changed from using rail cars to lorries, 6 years ago.
Recently, the employee fell of the plank. The sackbarrow, the sack (50kg) and the plank all landed on top of him, in that order. Result - a fractured femur.
Recommended action - As the injury was directly caused by a re-inforcing bar on the sackbarrow hitting the man's leg, remove the re-inforcing bar. The result of modifying this and all other sackbarrows in the warehouse wil be that, the next time someone falls off the plank, the re-inforcing bar will not be able to come in contact and injure the leg.
The man is still in hospital, all of the sackbarrows have been modified and the railway lines (and plank) are still there
As I said, don't laugh, there is a lesson in there somewhere.

I suppose it's a better solution than removing everybody's leg so that it can't get injured!

As it is a French accident, I am surprised the corrective action wasn't vetoed!!!!
