I seem to remember an acronym from my old NEBOSH days that went some thing like this!

FEW CHEFS MM Which relates to an inspection check list.
Fire,Electrical,Welfare,Chemical,Housekeeping,Environment,First Aid,Slips Trips & Falls,Manual Handling,Machinery. This list is not a panacea for all hazards relating to construction sites, but it is a good start. Once you get into the routine of a check list your halfway there, plus you can add to it as the site changes through its progression.

As for report writing I tend to keep it simple and to the point. following the findings paragraph I always include a piece about the benefits of maintaining a good safety culture compared to the cost of of an accident or safety failure, Accidents, insurance, fines, morale etc. If management can see a cost saving they are more likely to respond positively

I have a paper back book on report writing styles. If you would like to have it just mail me back with an address.

If you go to HSEs website, then to the free leaflets bit, then to Construction, there is a leaflet in there about report writing.

You will need to register to obtain the site Health & Safety toolkit but it is worth it as you will get extensive site safety and health check lists that can be extended to meet individual needs. It also has a checklist for concept, design and planning and a personal assessment checklist. All this and so much more for....free!
