As this will depend on:
a) The medical/health situation of the particular individual, and
b) What work it is that he is returning to,
it's hard to see how anyone can post anything that would help.
You need to speak to an occupational health provider or the individual's GP to understand his specific situation.

Peter,
I would suggest that no one would have an assessment that you could use sucessfully.
In cases such as this I have always formed a small group to assess the situation and design the retrun to full work process.
The members are normally myself +
The Person
The Occupational Health Practioner
The Area Manager.
The persons GP has always been consulted (with the persons permission of course)
I have always felt that this give a much better result for both the company and the person.
