I have only recently came into contact with this system of performance measurement in the H&S field and would like to hear from others (pros/cons of use) who have used or are using it now.

Phillip, not sure if Neil's paper will answer all of Kenny's question. Would rather refer to Dom Cooper's work on the "Reciprocal model of safety culture" Not available on line but I have a copy of his paper. (25 pages on the psychological roots of accident causation) including the metaresearch. (after which he achieved his current professorship. Haven't heard about the film of the book though)
