Would rather refer to Dom Cooper's work on the "Reciprocal model of safety culture" Not available on line but I have a copy of his paper. (25 pages on the psychological roots of accident causation) including the metaresearch. (after which he achieved his current professorship. Haven't heard about the film of the book though) Merv
