L140 Hand-arm Vibration Control of Vibration Regs 2005.pdf It's sister guide on whole body aslo has one but this tends to be less of a problem in most areas. INDG 362 availible free on the HSE website will give you a good hint as to what needs to be included in your form. Have fun, Andy
